OFFICIAL 2011 TEAM REGISTRATION FORM

The last day to turn in forms is August 8th. Any team that does not submit a
registration form by that date will not be allowed in the league this season.
If possible, please submit forms immediately. Turning in your forms early
is a huge benefit for the schedule makers. If we can start working on
schedules before August 8th, we can deliever more accurate schedules sooner.
Please submit one form for each team that your school will have. For example,
if you have one(1) JV and one(1) Varsity team, you should be submitting two(2) forms.
The entrance fee is $120.00 per team. This should be paid when you turn in this
form (by check made payable to Diocesan Girlsï¿½ Volleyball League).
You may mail the forms and payment to the address below. Do not wait until the last
minute to mail something, though, because if the form does not
arrive by August 8th your team will not be allowed in the league.
We will happily accept scans of the forms sent via email to sethrunatz@yahoo.com.
